Transfer In Registration

Online Form

Documents Required for Renewal of Registration

1. Filled Application Form

2. Self-Attested Copy of Degree (B.V.Sc. & A.H.)

3. Old Registration ID Card (Original)

4. Passport Size Photograph

5. Prescribed Fee is Rs. 1515 only.

6. If registration has expired please add late fee Rs. 200/ per month.
